BLUE STAR CARD DEPLOYMENT DISCOUNT CARD APPLICATION



If you are a Spouse of a deployed Soldier, you are eligible for the MWR Blue Star Card.  This Card will entitle you to Discounts and Special offers.
Please fill out the form below and submit it.  Then bring a copy of your Deployment Orders to one of the following ACS locations.
 (Please wait at least (2) working days after submitting your registration form before coming to ACS to get your pass, so we have time to input your information. Cards at ACS Schofield Barracks will be printed on the spot. Cards at the ACS Fort Shafter will be sent to ACS Schofield for printing and will be mailed to your house or can be picked up at a later date at Ft. Shafter ACS.)
